chenqingqing ade1f84409 fix(bt/controller): fixed several controller issues ...

- fix the start time in scheduling item
- stop inquiry ASAP when response number reached
- add support for hold mode
- stop handshake when acl link is already exist with same bdaddr
- fix AFH state after successful role switch
- return correct failure reason to avoid redundant "not accepted"
- fix build error when building rom code
- fix conflict between sniff and sniff subrate
